The World Health Organization (WHO) has reported 2,501 new confirmed mpox cases, including 12 deaths, across 44 countries worldwide in October 2025, highlighting continued global circulation of the virus and a growing risk of community transmission in multiple regions.

According to the latest WHO multi-country outbreak situation report, all known clades of the monkeypox virus (MPXV) remain in active circulation. Health experts warn that if outbreaks are not swiftly controlled, sustained human-to-human transmission could spread further into communities.

Africa Reports Highest Burden of Active Mpox Transmission

The WHO said that between October 12 and November 23, 21 African countries reported active mpox transmission, accounting for 1,734 confirmed cases and 10 deaths.

The countries reporting the highest number of new infections include:

Democratic Republic of Congo

Liberia

Ghana

Kenya

Uganda

Notably, Mali reported mpox cases for the first time, marking a new addition to affected nations.

Clade Ib Mpox Cases Spread Across Multiple Regions

The report confirmed that new imported cases linked to clade Ib MPXV were detected among travellers in:

Belgium

Germany

Greece

United Kingdom

Between September 5 and November 24, a total of 43 new confirmed clade Ib mpox cases were reported across six WHO regions, outside areas with previously established community transmission.

Community Transmission Detected in Six Countries

Alarmingly, in four WHO regions — the Americas, Southeast Asia, Europe and the Western Pacific — 24 of the reported cases had no recent international travel history, indicating local community transmission.

As a result, the WHO now considers the following countries to be experiencing community transmission of clade Ib MPXV:

Italy

Malaysia

Netherlands

Portugal

Spain

United States

In addition, several travel-related cases continue to surface globally, with household contacts and sexual partners emerging as secondary cases.

WHO Assesses Risk and High-Risk Groups

Among the 43 clade Ib cases recorded, 22 were reported among men who have sex with men (MSM). Other cases were associated with travel to regions reporting active outbreaks.

However, the WHO clarified:

> “The public health risk posed by clade Ib MPXV is assessed as moderate for men who have sex with men with new and/or multiple partners, and low for the general population.”

The overall case fatality ratio (CFR) for October stood at 0.5 per cent, indicating relatively low mortality but sustained transmission.

What Is Mpox and How It Spreads

Mpox (monkeypox) is a viral infectious disease that spreads mainly through close physical contact, especially skin-to-skin contact. Common symptoms include:

Fever

Swollen lymph nodes

Muscle aches

Sore throat

Skin rash

Back pain

Health authorities continue to stress early detection, isolation, contact tracing and targeted vaccination to prevent wider community spread.
